PD TeleCare Provides Supplemental Support for Your PD Patients on Automated Peritoneal Dialysis (APD)
PD TeleCare provides proactive technical support to patients and actionable insights to clinicians. For patients with Sharesource enabled cyclers, Baxter’s PD TeleCare service team remotely monitors patient treatment data during the first 90 days of a patient's PD therapy.

How Does PD TeleCare Work?
A dedicated Baxter PD TeleCare Nurse reviews Sharesource alert and alarm trends from Amia and Homechoice Claria APD cyclers to identify if a patient, who is in the first 90 days of therapy, needs additional technique or training support.
As training opportunities are identified, a Baxter Technical Support Representative will call the patient with training and technique related support.
In parallel, the PD TeleCare Nurse notifies the patient’s PD clinic nurse of potential clinical issues and the clinic PD nurse decides appropriate clinical intervention. The clinic nurse also receives a monthly report summarizing all PD TeleCare interactions with patients.

Sharesource is a globally available two-way digital health platform allowing you to remotely monitor and manage patients from your clinic or anywhere you have access to internet, including the ability to change a patient’s prescription setting.
INTENDED USE
The Sharesource portal is intended for use by healthcare professionals to remotely communicate new or modified treatment parameters with compatible dialysis instruments and transfer completed treatment data to a central database to aid in the review, analysis, and evaluation of patients’ historical treatment results. This system is not intended to be a substitute for good clinical management practices, nor does its operation create decisions or treatment pathways.
